Licensed Vocational Nurse

Job Summary

The Licensed Vocational Nurse is responsible for providing quality home health services to assigned patients under the direct supervision of the Clinical Supervisor. Through competent application of nursing processes and use of accepted best practices, provides quality home health services under the clinical supervision of a Skilled Professional. Ensures compliance with federal, state, and local regulations and agency policies.

Job Duties & Responsibilities

  1. Provides nursing services:
    • Follows the individualized Plan of Care
    • Assists with ADLs
    • Safely administers medication
    • Assists patients in self-care techniques
    • Prepares equipment and materials for treatment
    • Provides skilled interventions focused on patient outcomes
    • Reports changes in patient status and progress
  2. Approaches care systematically:
    • Implements individualized care plans
    • Uses teaching skills based on patient needs
    • Communicates changes to physicians and team members
    • Educates patients and families on community resources
  3. Maintains documentation:
    • Uses EHR to document care
    • Prepares notes and clinical forms timely
    • Identifies potential referrals
  4. Ensures patient rights:
    • Respects cultural/spiritual differences
    • Uses communication frameworks: AIDET, SBAR, Teach-Back
    • Maintains confidentiality and privacy
  5. Provides professional and compassionate care:
    • Demonstrates professionalism and teamwork
    • Participates in staff development
    • Teaches other personnel as assigned
  6. Promotes safety:
    • Follows infection control practices
    • Uses equipment safely
    • Identifies risks and promotes a safe environment
  7. Performs other duties as assigned.

Supervisory Responsibilities

May supervise Home Health Aides under direct clinical supervision with competency validation.

Competencies

  • Proficient in Microsoft Office Suite
  • Accurate documentation in Electronic Health Record
  • Strong communication, clinical judgment, and observation skills
  • Self-directed and organized
  • Flexible, tactful, and cooperative

License, Education, and Experience

  • LVN license in California
  • One year of clinical experience preferred (home health)
  • Meets all continuing education and documentation requirements

  • TB test & health clearance
  • Credentialing process completion
  • Valid CA driver’s license and clean record
  • Current CPR certification
