Physical Therapist Assistant

Job Summary

The Physical Therapist Assistant provides quality home health services to assigned patients under the direct supervision of the Therapy Lead. Through the application of physical therapy practices and best standards, ensures compliance with all regulations and agency policies.

Job Duties & Responsibilities

  1. Provides PT services under supervision:
    • Implements direct PT care
    • Coordinates care and case management
    • Follows physician-approved plan of care
    • Documents care and communicates changes
    • Performs preventative and rehab procedures
  2. Approaches care systematically:
    • Follows individualized plan of care
    • Uses teaching techniques appropriately
    • Educates patients and families on resources
    • Reports condition changes promptly
  3. Maintains documentation:
    • Prepares accurate clinical notes
    • Uses agency software for documentation
    • Identifies and refers potential clients
  4. Protects patient rights:
    • Honors individual, cultural, and spiritual values
    • Practices confidentiality
    • Uses communication models: AIDET, SBAR, Teach-Back
  5. Delivers responsible and professional care:
    • Demonstrates professionalism and competency
    • Engages in continuous development
    • Teaches others and promotes team collaboration
  6. Promotes safety:
    • Follows safety protocols and uses equipment properly
    • Recognizes and reports unsafe conditions
    • Prevents accidents and promotes safe environments
  7. Performs other duties as assigned.

License, Education, and Experience

  • Licensed Physical Therapist Assistant in California
  • At least one year of clinical experience preferred
  • Responsible for meeting all continuing education requirements

  • TB test & health clearance
  • Credentialing process completion
  • Valid CA driver’s license and clean record
  • Current CPR certification
